Infection with these organisms is most commonly found in the urinary tract although the normal environment of these organisms is usually the intestinal tract (where they are harmless), soil, or water.  Most often these infections occur in patients who are hospitalized, especially in intensive care units, who also have indwelling devices such as catheters or who have undergone other invasive procedures.  Residents of long-term care facilities such as nursing homes are also at risk.  Unfortunately, and potentially frighteningly, there have been reports of these infections occurring in the community among individuals who are not hospitalized, although some of these people had been hospitalized before developing the infection or they had contact with someone who was, or who was recently, hospitalized.  This trend of infection from healthcare settings only to also include community settings is one that has been seen before with MRSA, methicillin-resistant Staphylococcus aureus.

These organisms have been found in hospital environments including hand washing stations and sinks, as well as in other environments.  This makes it easy to transfer organisms from one patient to another, and potentially to visitors as well.  The presence of these organisms generally represents a failure to adequately clean and disinfect surfaces and facilities and may particularly represent contamination with fecal or urinary material.  It is essential that these organisms be eradicated from the hospital and other healthcare environments, and with the emergence of community acquired cases, it also increasingly desirable to eliminate their potential presence from other settings such as schools, gyms, and even homes, anywhere where people congregate and might potentially shed infectious organisms in bodily secretions.
